Gapapentin For Neuropathic Pain
To treat postherpetic neuralgia and partial onset seizures, the Food and Drug Administration has approved gabapentin (Neurontin). Clinical investigations on patients with diabetic neuropathy or postherpetic neuralgia have shown that daily doses of gabapentin between 2400 and 3600 milligrams (mg) are equally effective as doses of tricyclic antidepressants or carbamazepine in relieving pain.
The growing evidence supporting its efficacy in treating neuropathic pain has led to its widespread application. Gabapentin's mechanism of action and site of action remain mysterious. Gabapentin is well-tolerated, has a wide dosing range, has few drug interactions, and does not necessitate regular blood tests for its administration. However, for certain people, the expense may be prohibitive. Patients who would benefit from gabapentin should have a well-defined case of neuropathic pain, manifesting as piercing, stabbing, lancinating, or burning pain that travels along a nerve root (radicular) or in a stocking/glove pattern.
Dosing Adults Initial doses of Gabapentin 300mg Tabs are often raised by 100 mg to 300 mg every 1-3 days until an effective level is reached. For the first two days, you may take 300 mg at bedtime, 600 mg twice daily on days three through seven, and 1,200 mg three times daily on days eight and beyond. The effective daily dose is typically between 900 and 3600 milligrams (mg), spread over three doses. In senior patients, titration should be done more slowly. To avoid withdrawal seizures, tapering off gabapentin must be done gradually over at least a week.
The effectiveness of this treatment for childhood neuropathic pain is not well-studied. When other pain medications, including analgesics, are too drowsy for a child with neuropathic pain, the American Pain Society suggests trying gabapentin instead. Typically, their dosage ranges from 8 mg/kg/day to 35 mg/kg/day, and the starting dose is 2 mg/kg/day.
Warning signs of an allergic reaction to this medication include a rash, itching, difficulty breathing or swallowing, or swelling of the hands, face, or lips.
